• This package is a one-hour phone discussion with Dr. Schwarzbein, addressing your medical concerns.
• We will provide you with a basic medical questionnaire that we recommend you complete (Not mandatory, but helpful to Dr. Schwarzbein).
• You will be responsible for providing any pertinent lab work or testing you have completed that you want addressed during your phone time.
