All of our forms are now available to you as PDF files you can open and use with the Adobe Acrobat Reader. After you click on a file name, it should open up automatically for you in the Acrobat Reader. If the file does not open, click here to go to Adobe's web site and download the plug in you will need. It is free and very easy to download and use:

Reader for Forms

You must have at least version 5 of the acrobat reader to read our forms. Older versions of the reader will not open the forms correctly. Please upgrade if you have an older version, or have any problems.

After installing the new version of Reader, we also recommend rebooting your computer.


It sometimes happens that you can open a form but not be able to print it. If you download the current Acrobat Reader and re-install it, this will fix this problem.


We have occasionally noticed a problem with the PDF plug-in and Internet Explorer. When the preferences in Explorer are set to open a PDF document WITHIN Explorer, it often stalls or "hangs up". This can be easily resolved by having Explorer download the PDF file directly to your hard disk, then opening the PDF file up yourself, using the Acrobat Reader.

To change the way Internet Explorer deals with a file do this:

In Internet Explorer – go to preferences > Receiving Files> File Helpers > scroll down to ".pdf" (they're alphabetically sorted on the right side) = Portable Document Format. Then click CHANGE.

Choose "view with application" instead of "view in browser". You may need to locate the application "Acrobat" or "Acrobat Reader" on your hard disk, using the Browse button. Then click OK, and try again.
